Frequently Asked Questions

Our therapists and staff are here to serve your child’s needs and support the family. For commonly asked questions on our office procedures and policies, please see below:

What do I need to bring to my child’s first appointment?

Please arrive at least 15 minutes early for your first appointment so you have time to register and go over the necessary paperwork. Feel free to download our Intake Form from the patient forms, fill it out ahead of time and bring it in with you.

Please arrive with your family insurance cards. In addition, it is very helpful if you take some time to write down questions you may have, common signs, symptoms and difficulties with activities your child has.

If you have any helpful prior reports, lists of medications and other medical or school documentation that may assist the therapist with getting to know your child’s  history, please bring those as well. In addition, please bring any necessary equipment or assistive devices that your child commonly uses.
